Parent Permission Form for Youth Ministry Programs

 

Dear Parent or Legal Guardian:

 

Your son/daughter is eligible to participate in a parish-sponsored activity requiring transportation to a location away from the parish premises.  Parents/guardians are responsible for transportation to and from events for those children in their care unless otherwise stated.  A parent can designate another parent/youth to transport his or her child.  This activity will take place under the guidance and supervision of employees/volunteers from St. Anne parishes.  A brief description of the activity follows:

If you would like your child to participate in this event, please complete, sign, and return the following statement of consent release of liability.  As a parent or legal guardian, you remain fully responsible for the actions and conduct of your child.

                                                         STATEMENT OF CONSENT                                                                         

 

I hereby consent to participation by my child (full name)                                                                   in the event described above.  I understand that this event will take place away from the parish grounds and that my child will be under the supervision of the designated parish employee/volunteer on the stated dates.  I further consent to the stated conditions on participation in this event, including the method of transportation.

 

In consideration of my child being allowed to participate in this field trip, I agree to indemnify and hold harmless and defend St. Anne Parish any and all affiliated organizations, their employees, agents and representatives, including volunteer and other drivers, from any and all claims, including negligence, arising from or relating to my child’s participation in this field trip.  This indemnification and hold harmless and defense agreement does not apply to claims for intentional misconduct

Dear Parents and Youth,

 

Here are some guidelines for the trip:

Please KMart parking lot (on Euclid Avenue) by 5:15 a.m. Bus will pullout of Kmart parking lot promptly at 5:30 am; we will not wait for stragglers.

Bus will leave Cedar Point  promptly at 7:00 p.m. You must report to the bus by 6:45 p.m. Bus will return to Kmart parking lot at approximately 12:00 p.m.

All youth will have to report to adult chaperones at check-in times during the day.

Food and beverages are permitted on the bus. No glass containers are permitted. Outside food, beverages or coolers are not allowed in the park. You are responsible for providing or purchasing your own food.

Shirts, shoes and shorts/pants are required in the amusement park at all times. To maintain the family atmosphere a dress code is enforced. Please do not wear clothing that is generally considered offensive.

Lockers are available at the entrance to the amusement park

Take spending money if you’d like.

There must be at least one watch per group of youth.

Each youth must have a signed permission form and a medical treatment release form. Please turn in both at time of payment.

 

Reservations will be on First Come Basis on permission slips accompanied by full payment. Payments will be received at St. Anne Parish Rectory, 315 W Center St, Linwood, MI. (697-4443). The office is open M - F from 9:00 – 5:00.

 

No money will be refunded unless we are able to replace you.
